Physical Therapist
Campbell Hall is seeking a dedicated Physical Therapist to join our therapy department and help our residents regain mobility, independence, and confidence. You'll work with people at all stages of recovery—from post-surgical rehabilitation to managing chronic conditions—in a collaborative, person-centered environment.
As a Physical Therapist, you're a core member of our interdisciplinary care team. You'll conduct comprehensive assessments, design individualized treatment plans, and lead hands-on therapy sessions that make a real difference in residents' daily function and quality of life. Your work directly supports residents' goals to return home or maintain the highest level of independence possible.
What you'll do
- Perform initial and ongoing physical therapy evaluations, including range of motion, strength, balance, and functional mobility assessments
- Develop and implement individualized treatment plans based on resident goals and medical history
- Conduct direct therapy sessions—gait training, therapeutic exercise, balance and fall-prevention work, and functional mobility training
- Monitor resident progress, adjust treatment as needed, and document outcomes in the medical record
- Collaborate with nursing, social work, and the care team to coordinate discharge planning and community resources
- Communicate regularly with residents and families about therapy progress and realistic expectations
- Stay current with evidence-based practice and participate in continuing education
- Maintain compliance with HIPAA, infection control, and facility safety protocols

Requirements
- Current, active New York State Physical Therapist license (PT)
- Bachelor's degree minimum in Physical Therapy (DPT or equivalent); graduation from an accredited PT program
- Ability to work full-time hours as scheduled
- Physical ability to perform direct patient care, including standing for extended periods, bending, lifting, and assisting residents with transfers and mobility tasks
- Negative tuberculosis (TB) test or proof of immunity per facility policy
- COVID-19 vaccination status per facility policy
- Excellent written and verbal communication skills
- Demonstrated ability to work collaboratively with interdisciplinary teams and communicate effectively with residents and families
