As an LPN, you'll be a core member of our nursing staff, delivering skilled patient care, collaborating with RNs and CNAs, and ensuring our residents receive the attention and clinical oversight they deserve. • Assist with wound care, catheter management, and other nursing procedures as ordered
  • Collaborate with CNAs and interdisciplinary team members to coordinate daily resident care and comfort
  • Communicate with families and physicians regarding resident status and care updates
  • Maintain accurate, timely medical records and nursing documentation
  • Monitor and respond to resident requests and changes in condition throughout your shift
  • Support infection control and safety protocols, including proper PPE use and hand hygiene

What you'll bring

  • Clinical competence and attention to detail in patient assessment and medication administration
  • Strong communication skills with residents, families, and the care team
  • Compassion and patience when working with older adults and those with complex medical needs
  • Reliability and professionalism, even in a flexible per diem role
  • Ability to manage time and prioritize tasks across a busy shift

Why join us

  • Per diem flexibility—choose shifts that fit your schedule across days, evenings, and nights
  • Supportive nursing team that values collaboration and professional growth
  • Meaningful work caring for residents and families who depend on skilled, compassionate nursing
  • Opportunities to maintain and expand your clinical skills in a long-term care setting
  • Safe, clean environment with clear protocols and strong infection control practices

Requirements

  • Current, active New York State Licensed Practical Nurse (LPN) license
  • High school diploma or GED
  • Minimum 1 year of nursing experience (clinical setting preferred)
  • Availability to work flexible shifts including days (7–3), evenings (3–11), and nights (11–7)
  • Ability to lift and transfer residents (up to 50 lbs with assistance) and stand for extended periods
  • New York State fingerprint clearance (BCI/DCJS) and background check clearance
  • Negative tuberculosis (TB) test or proof of immunity within the past 12 months
  • Compliance with facility vaccination policies, including COVID-19 as per New York State requirements
